Membership Terms

All transactions are considered final. We do not have a long-term contract, and membership may be cancelled at any time with a 30-day written notice. Following initial payment, your payment source on file will be automatically charged on your monthly/yearly anniversary, as determined by your billing preference (monthly vs yearly). We are willing to work with our patients, but if payment is not received a nonpayment policy will be as follows: 1 month of nonpayment will result in an INACTIVE membership status and full payment will be required before any additional service will be delivered; 2 months of nonpayment assumes SELF-TERMINATION, at which time membership will be cancelled without an option to renew. Recurrent nonpayment issues may result in membership termination. A terminated member may not have the opportunity to renew their membership.
